原发肿瘤(乳房、结肠、肺、前列腺癌)在形成生长早期,会释放细胞进入血液循环。通过各种技术可以富集检测这些循环肿瘤细胞的物理和生物性质。
循环肿瘤细胞,Circulating Tumor cell,简称CTC。CTC释放点:原发肿瘤和转移肿瘤。
CTC死亡后还会释放出循环肿瘤DNA,Circulating Tumor DNA,简称ctDNA。
CTCs和ctDNA的分析是液体活检诊断的基石。
CTC和ctDNA的临床应用
肿瘤检测:CTCs和ctDNA应用肿瘤的早期检测很受关注,面临的挑战是目前分析的特定性和敏感度。
预后评估:多种肿瘤已经实现预后评估,尤其是乳癌。
指导系统治疗:
病人亚群分类(stratification)。
CTC研究进展
CTC研究是很活跃的领域,截止2015年12月发表在PubMed的文献有17000篇。随着循环游离肿瘤DNA分析的发展,zui近才开始使用“液体活检”这个条目。分子水平的灵敏分析方便研究者从血浆中筛选特定肿瘤突变的ctDNA,因此ctDNA和CTC成为富有竞争力的生物标志物。不过建议把ctDNA和CTC作为两种互补的信息资源,根据应用区分。文章简单讨论生物和检测技术,重点是ctDNA和CTC的肿瘤临床应用。
